Whole-Person Health Care in Warsaw, IN
Our Mission in Warsaw, IN
A Membership Clinic Built for Real People
Legacy Family Health Care was built around a simple but powerful idea: people are more than their diagnoses. Our whole-person health philosophy means we look at the full picture of your physical, mental, and lifestyle health together, not in separate silos. Instead of rushing through a 10-minute appointment to address one complaint, we take 45 minutes at intake to understand who you are and what your body actually needs. That kind of depth is how we find root causes rather than just managing symptoms. Lindsay M. Kreps, MSN, APRN, FNP-C, brings more than 20 years of clinical experience to every patient relationship, with a particular focus on diabetes and chronic disease management. In February 2025, Rhonda Hoskins, MSN, APRN, FNP-BC, joined the practice, bringing specialized expertise in psychiatric nursing and medication management. Rhonda sees patients Monday through Wednesday, expanding Legacy's ability to support both physical and mental health under one roof.

How We Work
A Membership Clinic Built for Real People
Legacy Family Health Care operates as a membership-based, self-pay clinic. For $80 per month, members receive their annual wellness visit, annual labs, and covered standard office visits, with no insurance billing for office visits and no surprise costs hidden in the fine print. This model was designed specifically for patients who are uninsured, carry high-deductible plans, or simply want to know exactly what their care costs before they walk in the door. Our clinic is appointment-only, which means every visit is calm, focused, and unhurried. We serve patients from Warsaw, Winona Lake, Leesburg, Syracuse, Goshen, Fort Wayne, Plymouth, Wabash, and throughout Kosciusko County. We also proudly support both English and Spanish-speaking patients, because quality care should never depend on the language you speak.

Who is Lindsay Kreps and what does she specialize in?
Lindsay M. Kreps, MSN, APRN, FNP-C, is the owner and lead provider at Legacy Family Health Care in Warsaw, IN. She brings more than 20 years of clinical experience to the practice, with a particular focus on diabetes and chronic disease management. Her approach centers on treating the whole person rather than a single symptom, which means she takes time at every visit to understand the full picture of a patient's health. Her 45-minute intake appointments reflect that commitment to depth and thoroughness.Who is Rhonda Hoskins and when did she join the practice?
Rhonda Hoskins, MSN, APRN, FNP-BC, joined Legacy Family Health Care in February 2025. She specializes in psychiatric nursing and medication management, making her the primary provider for patients seeking mental health medication support at the clinic. Rhonda sees patients on Mondays, Tuesdays, and Wednesdays, and her addition to the team expanded Legacy's ability to offer both physical and mental health care under one roof in Warsaw, IN.What does the $80 per month membership include?
The $80 per month membership at Legacy Family Health Care covers your annual wellness visit, annual lab work, and standard covered office visits. There is no insurance billing for office visits, so the cost is straightforward and predictable. Certain procedures, such as mole removal or abscess drainage, are priced separately from the monthly membership fee. This model is designed to give patients, especially those who are uninsured or carry high-deductible plans, clear and consistent access to primary care without financial surprises.Does Legacy Family Health Care accept walk-in patients?
No. Legacy Family Health Care is strictly an appointment-only clinic. This is an intentional choice that allows the care team to give every patient their full, undivided attention without the rushed atmosphere common in walk-in settings. Appointments must be scheduled in advance. This structure supports the longer, more thorough visits that are central to the clinic's whole-person health philosophy.Does the clinic offer care in Spanish?
Yes. Legacy Family Health Care provides care for both English and Spanish-speaking patients. Language access is considered a core part of delivering truly patient-centered care, and communicating in a patient's primary language helps build trust and reduces the risk of misunderstanding during clinical conversations. This bilingual support is one of the ways Legacy serves the broader Northern Indiana community, including patients from Goshen, Fort Wayne, and throughout Kosciusko County. How does Legacy's model differ from a traditional insurance-based clinic?
Traditional insurance-based clinics are typically structured around volume, which often results in short appointments, rotating providers, and care focused on a single presenting symptom. Legacy Family Health Care operates outside of that model. As a membership-based, self-pay clinic, Legacy does not bill insurance for office visits. Patients pay a flat monthly fee and receive longer appointments, consistent access to the same providers, and a care approach that looks at root causes rather than surface-level symptoms. What types of conditions and services does Legacy Family Health Care cover?
Legacy offers a wide range of services, including chronic condition management for diabetes, high blood pressure, high cholesterol, asthma, and thyroid conditions; primary care and wellness visits; DOT and CDL physicals; mental health medication management; same-day support for common illnesses; minor in-office procedures; in-house lab testing; in-house pharmacy and medication refills; coordination of care with specialty providers; weight loss support; and telehealth follow-up visits for established patients. What are the clinic's hours?
